This Website Privacy Policy explains how ClearPath Psychiatry LLC handles information connected with this public website. It is separate from our HIPAA Notice of Privacy Practices, which explains the use and disclosure of protected health information and your patient rights.

Information collected through this website

Our website is hosted by Squarespace. Squarespace may process technical information such as your IP address, browser and device details, and page requests to deliver and protect the website. We have disabled the Squarespace visitor Activity Log in our site settings. This does not eliminate all technical processing by the hosting provider.

This website directs appointment requests and client access to SimplePractice. It does not provide a medical intake form on the public pages. If you contact us by telephone or email, we receive the contact information and content you choose to share so we can respond to your inquiry.

Cookies and analytics

We have configured Squarespace to restrict nonessential cookies and have left the cookie banner off. Necessary cookies may still be used to operate the website. These settings do not mean that the website collects no technical information, and they do not control cookies or data processing on external services such as SimplePractice. How information is used and shared

Website and inquiry information is used to operate and protect the site, understand site use, respond to questions and help visitors access scheduling or practice services. Information is handled by service providers that support these functions, including Squarespace for hosting and analytics and our business communication services for phone and email.

Information may also be disclosed when required by applicable law or a valid legal process. Any information that qualifies as protected health information remains subject to applicable health privacy requirements and our separate Notice of Privacy Practices.

Appointments and the Client Portal

When you follow an appointment or Client Portal link, you leave this website and enter SimplePractice. Information you submit there is handled through that service and our practice’s patient privacy procedures. Review the privacy information provided in the portal. This Website Privacy Policy does not replace patient consent forms or the Notice of Privacy Practices.

Please use your Client Portal for care-related messages and forms shared by the practice. Do not send medical records, symptoms, payment card details or other sensitive information through ordinary email.

Your choices and privacy questions

You may browse the public pages without sending us an inquiry. You may control cookies through your browser and contact us with questions about information you provided. Requests to access, correct or delete information are considered subject to applicable law and any record-retention obligations; some records cannot be deleted on request.

Security and retention

No website, email system or internet transmission can be guaranteed completely secure. Avoid sending sensitive information through public or unsecured channels. Information may need to be retained to respond to inquiries, support practice operations or meet applicable legal and recordkeeping requirements. Clinical records are handled under the practice’s separate patient privacy procedures.

ClearPath’s services described on this website are intended for adults ages 18–65. This website is not intended to collect information from children under 13.
